I. TITLE: Community Connections Trainer
JOB CODE: PARA 502
II. SUMMARY:
An employee in this classification, under the supervision of a Community Inclusion Coordinator, provides on-site assessment, training, and support to individuals to assist them to connect to their community.

III. DUTIES:
60% Develops opportunities for individual(s) and/or group(s) to connect to their community through assessment and exploration of areas of ability, interest, and skill.
Connects individuals to community settings and provides support in the setting including specific skill training, adjustment to the environment, identifying and developing natural supports, and modeling appropriate behavior, work skills and social interaction.
Facilitates interaction and/or communication between individual(s) and other community members in order to enhance integration.
Utilizes fading techniques to withdraw staff and support services when appropriate.
Assists with planning, developing, promoting, and implementing recreational activities, Special Olympics sports/league, recruitment of volunteers, travel training, as well as other events and activities that further community integration and employment for the individuals we support.
Establishes and maintains collaborative relationships that include supporting and consulting with community members, employers, providers, volunteers, other professionals, families, and residential staff.
Complies with all applicable rules, regulations, laws and Board policies and procedures.
Required to transport individuals supported by the Board and drive an agency vehicle.
Works as a member of an interdisciplinary team to review desired outcomes and action steps as well as identify barriers to community integration, community employment including individual adaptive behaviors, parent and/or caretaker concerns, transportation possibilities, the individual’s expressed willingness to participate and other relevant factors.

Provides on-going support to individuals in community.
Requires use of public transportation systems throughout our community.
Participates in team meetings for each individual as requested.
Maintains regular communication with Service & Support Specialists and other department staff regarding individuals progress, further training needs, and exchange information.
Accurately records activities and documentation on individuals supported, and completes required paperwork within identified timeframes.
Disseminates information to agency staff and individuals about potential recreational activities, Special Olympics sports/leagues as well as other events and activities that further community integration for the people we support.
Collaborates with Board staff as well as community partners to facilitate recreational programs to meet the needs of the individuals we support.
15% May arrange transportation to and from community site for individuals supported.
May be required to orient staff assigned to the department.
5% Performs other related duties as required.
IV. EQUIPMENT OPERATED:
Personal computer, applications software, basic office equipment, cell phone, recreational equipment.
Agency vehicles.
V. WORKING CONDITIONS:
Position may involve exposure to individuals with medical and behavioral risks. Exposure to inclement weather conditions. Safety hazards associated with sporting activities.
VI. KNOWLEDGE, ABILITIES AND SKILLS:
Knowledge and use of public transportation systems.
Knowledge of supported employment practices and resources
Knowledge of community inclusion practices and procedures.
Ability to plan, organize, and implement community and/or recreational activities.
Ability to provide training/education to those supported in individual or group settings.
Knowledge of Special Olympics programs.
Skill in written and verbal communication.
Observation, evaluation, and training skills.
Ability to quickly learn job tasks at various work sites in preparation to support and provide on-the-job training to individuals.
Ability to establish and maintain harmonious relationships.
Complies with all applicable laws and board policy regarding confidential information.
Effective organizational skills.
VII. MINIMUM QUALIFICATIONS, EDUCATION AND EXPERIENCES:
A. Education:
High school diploma or GED.
B. Experience:
One (1) year teaching or training experience within one of the following: 1) a community based setting 2) an integrated volunteer worksite 3) school setting, and at least one (1) year of experience working with individuals with developmental disabilities required.
C. Credentials:
1. Certification:
Certification as issued by the Ohio Department of Developmental Disabilities (DODD).
2. Registration:
N/A
3. Licensure:
Valid driver’s license.
D. Ancillary Training:
1. Adult CPR/AED and First Aid
2. Annual Van Driver Training
3. Mandatory training as required by LCBDD, DODD, and OOD.
E. Other:
1. Required to pass basic Microsoft Word and Electronic Mail proficiency tests.
2. Ability to lift up to 25 pounds.
3. Requires own transportation.
4. Will require evenings and weekends, and locations convenient for persons/families supported.
Must be qualified to perform all duties assigned to this position.
